Title IX Incident Report Form
College’s Policy and Procedure on Discrimination, Harassment, and Title IX Sexual Harassment
Please use this form to file reports of discrimination, harassment, sexual harassment, sexual and domestic misconduct (which includes sexual assault and dating violence), stalking, and retaliation. If you are unsure whether an incident is prohibited or covered by Title IX, you are encouraged to fill out this form.
This form may be filled out by the person experiencing harm, a responsible employee, or any other person who witnesses or has knowledge of the prohibited conduct. If you are filling out this form regarding conduct that happened to someone else, please understand that we may not follow up with you unless you are directly involved in this case, but we will always follow up with the people who were directly affected by the behavior described in this form.

Please fill out the form completely and honestly. The information in this report will be provided to the Title IX Coordinator and will be kept as private as possible; however, it may trigger a Title IX investigation.
Anonymous Reports may be made anonymously via telephone or in writing. Depending on the level of information available about the incident or the individuals involved, Davis & Elkins College’s ability to respond to an anonymous report may be limited. Davis & Elkins College will, however, take whatever steps it deems appropriate and in the best interests of the overall Davis & Elkins College community, consistent with the information available.
The Campus Conduct Hotline is a service utilized by Davis and Elkins College that allows individuals to anonymously report concerns that may impact the campus community to a trained specialist. Callers will be given a case number, and it is optional to identify yourself or to provide contact information. Concerns will then be submitted to the campus administration for Investigation. Using the provided case number, callers can check the status of the Investigation and learn of any actions that may have been happen.
